We are accepting new patients.
Patients are seen by appointment, which includes same day appointments. However, walk-ins are welcome and will be worked into the schedule between the patients who have appointments.
Please notify the office, at least 24-hours in advance, if you need to cancel or reschedule your appointment.
Payment for Services
All payments, including co-pays and deductibles are due at the time of service. Please provide a current copy of your insurance card. If there are any services that are not covered, partially or fully, by your insurance company, you will be responsible for the balance.
Cash, credit cards and checks are acceptable forms of payment. Please note that there will be a $30 fee for any returned check.
Failure to pay your known co-pay or other payments owed at the time of your visit is subject to an additional late fee of $25.
Any balances that are not paid after several requests by the billing specialist, will be referred to a collection agency.
Medications are refilled during office visits. However, there may be times when you notice that you are running out before your next appointment. Please call for any medication refills at least 3 business days before you are out of medicine. This gives us time to make sure the refill is still appropriate and get the necessary information to the pharmacy. We do make every effort to accommodate your refill request by the next business day.
Refill requests are only handled during business hours.
Antibiotics and controlled substances, such as certain pain medicines and anxiety medicines, will not be refilled outside of an office visit.
Please call 911 for all life-threatening emergencies.
We encourage you to call the office if you have any questions about your medical care. If we are with other patients and the answering machine picks up your call, please leave a detailed message and we will get back to you within 24 hours.
Dr. Cameron is available for emergencies after office hours. Please call if you are having a medical problem and need advice as to what to do. She will not treat medical problems or refill prescriptions. Please call the office during business hours for treatment or medication refills. To reach Dr. Cameron after hours call (770) 271-8484. Inpatient Hospital Coverage
In the event that a patient requires admission to the hospital, Dr. Cameron has made arrangements with The Inpatient Medical Group, who will serve as the admitting physicians. They admit patients to Gwinnett Medical Center – both Lawrenceville and Duluth locations. Dr. Cameron will work closely with the physicians in this group to coordinate your care.
Form Completion: There is a $15 charge for all forms to be completed by our office. Additionally, any letters that require a provider signature will incur a free of $15. Please allow 5-7 business days for completed paperwork.
Medical Records: A copy of your medical records can be obtained with an advanced fee. This fee is $30 for up to 20 pages and then $0.50 for each additional page.
If you have tests performed at our office, most results will be received back in 3-5 business days. We will contact you with results. Some results will require an office visit or referral.
It is the policy of our practice that all physicians and staff preserve the integrity and the confidentiality of protected health information (PHI) pertaining to our patients. The purpose of this policy is to ensure that our practice and its physicians and staff have the necessary medical and PHI to provide the highest quality medical care possible while protecting the confidentiality of the PHI of our patients to the highest degree possible. Patients should not be afraid to provide information to our practice and its physician and staff for purposes of treatment, payment and healthcare operations (TPO). To that end, our practice and staff will:
- Adhere to the standards set forth in the Notice of Privacy Practices.
- Collect, use and disclose PHI only in conformance with state and federal laws and current patient authorizations as appropriate.
- Not use or disclose PHI for uses outside of the practice’s TPO, such as marketing, employment, life insurance applications, etc. without an authorization from the patient.
- Treat all PHI data as confidential in accordance with professional ethics, accreditation standards, and legal requirements.
- Not disclose PHI data unless the patient or legal representative has properly authorized the release or the release is otherwise authorized by law.
- Recognize that although this practice “owns” the medical record, the patient or legal representative of the patient has the right to inspect and obtain a copy of his/her PHI. In addition patients have a right to request an amendment to their medical record if they believe their information is inaccurate or incomplete.
All staff and physicians must adhere to this policy. Our practice will not tolerate violations of this policy. Violation of this policy is grounds for disciplinary action, up to and including termination of employment and or criminal or professional sanctions in accordance with our practice’s personnel rules and regulations.

If PHW billing staff determines that an overpayment has been made, the credit balance will be submitted to refund the appropriate party, regardless of whether a refund as been requested.
